Adams County receives federal funding to implement its Community Services Block Grant (CSBG) program, which supports efforts to reduce poverty and improve self-sufficiency for low-income residents. As part of this funding, the County is required to conduct a Community Needs Assessment and develop a Community Action Plan every three years to identify and address local needs, service gaps, and opportunities for improvement. To fulfill this mandate—and do more than simply check a box—Adams County partnered with Omni to lead a process that goes beyond compliance, centering community voices and lived experiences to guide meaningful change.

Omni worked closely with county leaders, service providers, and residents to deliver a justice-driven assessment that captured the realities of life in Adams County through data analysis, stakeholder engagement, and on-the-ground insights. The final report surfaces key challenges and disparities across housing, healthcare, safety, and economic mobility, while also offering clear, actionable recommendations to inform the County’s next Community Action Plan. This work lays the foundation for responsive strategies that help ensure public investments align with what communities actually need to thrive.
