Elbert County Community Health Assessment

As part of the Colorado Health Assessment and Planning System (CHAPS) process, Omni analyzed publicly available data to spotlight health trends and uncover gaps in local care—such as provider shortages and service access. To ground the findings in lived experience, Omni also designed and launched a community survey that captured what residents see as their biggest health needs and opportunities.

Paired with qualitative insights gathered by Elbert County Public Health, the result was a rich, multi-perspective snapshot of thecommunity’s health. The assessment now serves as a launchpad for strategies that are not only evidence-based, but deeply aligned with the voices and realities of Elbert County residents.
