Omni conducted a Community Health Assessment (CHA) andcorresponding strategic Community Health Improvement Plan (CHIP) for Lincoln County in Colorado to better understand the area’s most pressing health andenvironmental challenges. Drawing from youth and adult surveys alongside in-depth community interviews, the CHA offered a 360-degree view of local health needs and lived experiences. This process didn’t just examine data—it built the foundation for actionable insights rooted in community voice andcross-sector partnership.

The resulting CHIP for 2024–2029 charts a clear, measurable path forward. It outlines key priorities, identifies vital partners, and includes concrete action steps and tracking mechanisms to ensure progress isnot only envisioned but achieved. By incorporating a Capacity Assessment to evaluate existing strengths and gaps, the effort positions Lincoln County to build on what’s working, address what’s not, and move toward a healthier, more connected future.
