As part of the county’s Community Health Needs Assessment (CHNA), Omni led 24 key informant interviews with representatives from 15 sectors—including grassroots organizations, healthcare, housing, and law enforcement. Using the Mobilizing for Action through Planning and Partnerships (MAPP) 2.0 framework, the assessment centered the voices of those most impacted by inequities. Participants offered rich insights on community strengths, the role of physical and cultural assets, and the broader local and global forces influencing health. This approach ensured that the assessment captured not only data, but the lived realities behind it.

The qualitative findings were integrated into the larger CHNA report to directly inform the county’s Community Health Improvement Plan (CHIP). By grounding strategy in community experience, the assessment supports more responsive, place-based interventions. Omni’s role helped ensure that planning efforts reflect not just what communities need—but what they already know, value, and envision for their collective health future.
