Through the Colorado Health Assessment and Planning System (CHAPS) process, Omni designed and implemented a tailored community survey and facilitated focus groups—including youth and healthcare providers—to capture a wide range of perspectives on local healthneeds. These data gave voice to lived experiences and uncovered critical insights for planning.

The findings directly supported the development of Teller County’s Public Health Improvement Plan, equipping the county with awell-rounded, equity-informed foundation for submitting to the Office of Public Health Practice, Planning, and Local Partnerships.
